From Surgery to Full Return to Sport: ACL Reconstruction Recovery at McFerran Physio

Patient Background
The patient, a 28-year-old female, ruptured her ACL playing football and underwent surgical reconstruction using a hamstring tendon graft. Her long-term goal was clear:
“I don’t care how long it takes — I want to return to football with minimal risk of re-injury.”
She first came to see us two weeks after surgery, presenting with:
- Significant knee swelling
- Limited range of motion
- Crutches and brace use
- Pain and restricted mobility
Early Rehab Goals & Progress (Weeks 0–6)
Initial Goals
- Reduce swelling
- Achieve full knee extension
- Improve flexion beyond 90°
- Transition to walking without crutches
Treatment Focus
- Gentle soft tissue massage
- Passive and active-assisted range of movement work
- Wound care and dressing support
- Isometric glute bridges, calf raises, and light quad engagement
By the 6-week follow-up with her surgeon, she had:
- Fully weaned off crutches
- Achieved 100° of knee flexion

Strength & Stability Phase (Weeks 6–12)
We began:
- Modified squats using a high box to control depth.
- Exercise bike training with seat adjustments to promote flexion.
- Single-leg balance and proprioception
- Glute, calf, hamstring, and growing strength work.
- Progressive leg press training (closed chain quad focus).

Advanced Strength & Functional Loading (Weeks 12–20)
We began pushing harder:
- Introduced open-chain quadriceps training (single-leg knee extensions). Single Leg Extension Eccentric | Machine | Strength and Conditioning Exercises
- Started with isometrics.
- Gradually increased concentric and eccentric loading.
- Continued split squats, step-ups, lateral lunges, and full-depth squats.
- Integrated RDLs and progressed hamstring curls.
- Progressed single-leg balance and proprioception
Progress was closely monitored to ensure no swelling, pain, or adverse response at any point.
Plyometrics, Landing Mechanics & Agility (Weeks 20–28)
With strength benchmarks met, we introduced:
- Landing drills: double-leg, single-leg, lateral landings
- Jumping & hopping progressions:
- 2-leg → 1-leg → 1-leg lateral
- Speed and height gradually increased
- Straight-line running → full agility progression:
- 45°, 90°, 135°, and 180° change of direction
- Deceleration and sudden stopping drills
The patient responded exceptionally well, with no flare-ups or setbacks.
Final Phase: Testing & Return to Play (Months 6–9+)
We coordinated isokinetic testing with a local university to assess:
- Quadriceps and hamstring strength symmetry
- Right-to-left leg ratios
- Hamstring:quad balance on the injured leg
