Resolving Persistent Neck Pain in a Desk Worker: A Case of Postural Dysfunction and Muscle Weakness

Introduction
In today’s working world, neck pain from poor posture is one of the most common complaints among desk-based workers. But too often, people try to “stretch it out” or “live with it” rather than addressing the underlying issue: weak, overworked neck and upper back muscles. This success story highlights how McFerran Physio helped a 43-year-old professional resolve chronic neck pain in just a few sessions — without scans, surgeries, or long-term medication.

Patient Overview
This patient was a 43-year-old man with a desk-based job. He had been experiencing persistent neck and upper trap pain on the right-hand side for several months.

Self-Treatment Attempts Included:

  • Regular stretching
  • Use of heat
  • Postural changes (e.g. standing desk)
  • Painkillers
  • Informal soft tissue massage from his wife

Despite his efforts, the discomfort continued to interfere with his work and daily life. That’s when he came to McFerran Physio for a proper assessment.

Clinical Assessment

Key Findings:

  • No red flag symptoms (e.g. dizziness, double vision, neurological signs).
  • Full neck range of motion, but pain at end-range rotation and side flexion on the right.
  • Mild postural changes (forward head posture).
  • No muscle wasting, swelling, or weakness.
  • No signs of nerve involvement — reflexes, power, and sensation all normal.

This was a mechanical neck pain case related to postural loading and muscular imbalances — not a serious spinal issue. That in itself was an important reassurance for the patient, who had begun to worry about osteoarthritis or disc problems.

Diagnosis: Postural Neck Pain with Upper Trapezius Overload

The likely culprit was repetitive loading of the neck and shoulder stabilizers, particularly the upper trapezius, due to prolonged forward-head posture at the desk. While his posture wasn’t “terrible,” subtle forward translation over long periods can lead to overactivation of certain muscles and underuse of others.

Manual Therapy (Short-Term Relief)

  • Deep tissue massage targeting the upper traps and cervical extensors.
  • Dry needling/acupuncture to reduce tone and improve tissue response.

These techniques were used selectively across 2–3 sessions to create a window for rehabilitation.

Exercise Therapy (Long-Term Correction)

  • Chin tucks and retractions (starting without resistance, progressing to resistance bands). Cervical Retraction Band
  • Trapezius shrugs to activate and condition the upper traps in a controlled way.
  • Scapular retraction exercises to improve posture and support.
  • Postural education and ergonomic coaching to reduce aggravation at the desk.

Critically, we explained why these muscles needed strengthening, not just loosening. Strength provides resilience and reduces the likelihood of re-injury.

Patient Outcome

After just 2–3 treatment sessions, this patient:

  • Had resolved his neck pain entirely.
  • Felt confident managing his own posture and workload.
  • Incorporated light strengthening work 1–2x per week as maintenance.

He no longer worries about “what’s wrong with his neck” — instead, he understands the cause, the solution, and how to prevent flare-ups.
